The Knox County Schools Transition Office consists of the following personnel:

1. a transition coordinator whose responsibilities include but are not limited to facilitating transition planning through the IEP Team process for all students with disabilities before they reach the age of fourteen and continuing through their exit year,

2. a transition case manager who is both a Knox County Employee and state vocational rehabilitation counselors who is targeted, by way of a federal grant, to provide services for the "at risk population",

3. a transition secretary who maintains records and develops a database of services provided, and

4. various part-time job coaches who are employed as needed


The basic responsibilities of the entire transition team is to assist students with disabilities in transitioning from Knox County Schools to adult life in the direction of an appropriate outcome chosen from the following options: post secondary education, specific vocational training, integrated employment, adult service providers, independent living, or community participation. This is accomplished through the following continuum of services:

A. Direct services through on the job training and community based placement.

and/or

B. Consultative basis by providing teachers with assistance in establishing goals and objectives for the development of individual transition plans.

and/or

C. Networking through facilitating the linkage of students with appropriate adult service providers; either through their time of school eligibility or providing information for post secondary options.

The transition staff is also available as an informational and referral source for parents, students, and professionals to utilize for guidance in choosing an appropriate outcome and the educational component to reach that outcome. Videos, brochures, oral information, and transition services can be accessed by calling the transition office at 594-3604, Monday through Friday, 8:00 am until 4:00 pm or by email pittmanm3@k12tn.net.
